PHOTOGRAPHIC RELEASE

I, the undersigned, give my consent (and/or consent on behalf of, and as legal guardian for a minor child) to Nursing Spectrum, a division of Gannett Satellite Information Network, Inc., and anyone authorized by Nursing Spectrum, to use photographs taken of me (and/or the minor child). In consideration for this consent, Nursing Spectrum agrees to only use the photographs as described in this consent form.

I understand that the photographs taken of me (and/or of the minor child) may be published by Nursing Spectrum for editorial, promotional, marketing and advertising purposes and that Nursing Spectrum may make such photographs available to third parties for editorial, promotional, marketing and advertising purposes, and the publication and use of the photographs may be accompanied by a caption or other material that discloses my name and medical condition, if applicable (and/or the name and medical condition of the minor child).

Further, I understand that I will not have the opportunity to approve the photograph and accompanying materials in advance, and I relinquish and give to Nursing Spectrum all rights, title, and interest I may have in the finished pictures or negatives.

Finally, I hereby release Nursing Spectrum, its licensees and their respective parents, subsidiaries, affiliated companies, successors and/or assigns from any and all claims, demands, or causes of action that I (and/or the minor child) may hereafter have against Nursing Spectrum, whether for libel, invasions of privacy, or anything else, by reason of the use of the photographs described above or the information that is published to accompany the photographs.

By signing below, I certify that I have read the foregoing consent form and release, and I warrant that I fully understand its contents.
